45rpm Single Sleeve detail changes:

Type 5:

This sleeve was a green and white design, The top half had 'RCA' in orange on a green background, the bottom half had 'RCA' in green on a white background.
 

The front and back were almost identical except the back had foldovers down both edges with 'Made in England' printed in green on each foldover.
